Systems Based Practice -Tactics

Systems-Based Practice (SBP) is defined by ACGME as "an awareness of and responsiveness to the larger context and system of health care and the ability to effectively call on system resources to provide care that is of optimal value."

Teamwork
  1. Assures assignment of roles and responsibilities
  2. Helps prioritizes tasks and the setting of team goals
  3. Requests and gives help when needed
  4. Acknowledges contributions, biases and errors
  5. Demonstrates respect, support and understanding to team members
Advocacy
  1. Provides information for ongoing care
  2. Ensures continuity of care, when possible
  3. Identifies and ensure access to needed services
  4. Addresses professional concerns directly
  5. Describes impact of payment systems on access
Coordination
  1. Transfers care responsibly
  2. Engages others in decision-making and planning
  3. Effectively communicates decisions and plans
  4. Resolves deviations from established plans
  5. Participates in meetings to coordinate care
Technology
  1. Uses technology to aid efficiency and accuracy
  2. Uses existing patient care devices safely
  3. Seeks to understand and master new care devices
  4. Accesses resources materials to improve patient care
  5. Recognizes impact of technology on patient and family
Improvement
  1. Actively seeks feedback
  2. Offers constructive feedback
  3. Develops improvement goals
  4. Identifies problems that prevent optimum care
  5. Participates in improvement efforts (QA, PI)
Cost- effectiveness
  1. Knows the cost of commonly used medications and techniques
  2. Incorporates cost in to selection of techniques
  3. Can explain concept of value (individual and global)
  4. Recognizes differences in health care delivery systems
  5. Participates in the design/use of pathways/guidelines
Safety
  1. Recognizes medical errors and their impact
  2. Discloses near misses, lapses and mistakes
  3. Seeks to minimize errors
  4. Uses best evidence (EBM) when possible
  5. Participates in M &M and Root Cause Analysis meetings
